Every Cub Scout participating in any Scouting activity is required to have medical paperwork on file with the pack. Due to HIPAA requirements, we are not allowed to accept and store these documents in digital form unless they are encrypted.
Please print and fill this form out and hand it over to your den leader or cub master at the next den meeting or pack meeting if you have not already done so this year.
The form needs to be updated every year.
